Who: The Commission to End Homelessness, comprised of nearly 70 leaders of Hennepin County – representing federal, state, and local governments, businesses, non-profits, faith and philanthropic communities, along with homeless and formerly homeless citizens – spent 100 days creating the plan to end homelessness.

Where: Heading Home Hennepin focuses on ending homelessness across the City of Minneapolis and throughout Hennepin County.

What: Heading Home Hennepin is a 10-year plan to end homelessness based on proven models and national best practices.

The report contains six broad goals, 30 recommendations, and over 50 concrete action steps to end homelessness. Successful implementation of the plan will prevent homelessness whenever possible; expand outreach to get people off the streets and provide stable housing for men, women, and children and well as the support services they need to succeed.

When: By the year 2016, Heading Home Hennepin expects to reach its goal of providing safe, decent and affordable housing for all people in Minneapolis and Hennepin County, as well as ensuring that everyone has access to services and no one falls into homelessness.
